Advocate gave you the history. Advocate+ gives you what racism is doing right now — to bodies, to systems, to the people in your care.
You completed Advocate. You can trace the arc from slavery and colonization to the clinical encounter. You can name how the profession has pathologized, excluded, and experimented on people of color. You have the definitions, the frameworks, the historical orientation.
That foundation is everything — because what comes next demands it.
The times are not abstract. ICE raids are separating families. Protesters are being beaten and detained. Psychiatric coercion is being wielded against the most vulnerable. The carceral state is not receding — it is expanding. And the mental health system remains entangled in it.
Advocate+ is where history meets the present tense.
In Advocate, you answered: How did we get here? In Advocate+, you answer: What is racism doing right now — and what does that require of me?
You are not yet executing the full clinical framework. That comes in Shield. But by the end of Advocate+, you will understand the current landscape of harm deeply enough to see it operating everywhere — in bodies, in diagnoses, in the algorithms rationing care, in the systems your patients fear as much as their illness.
The Advocate+ Tier is the third tier of the Antiracism in Mental Health Fellowship, developed by Dr. Rupi Legha, Harvard-trained, double-board-certified psychiatrist and one of the leading voices on racism as a public health crisis.
Designed with practitioners in mind — and open to anyone ready to go deeper.
BY THE END OF ADVOCATE+ YOU WILL BE ABLE TO:
- Recognize how the mental health system is structurally entangled with the carceral state — and how your clinical decisions are never neutral within it
- Explain how racism causes physiological harm at the cellular level — cortisol surges, weathering, epigenetics, allostatic load — and use that knowledge in clinical advocacy
- Identify and dismantle rigged discourses like health disparities, access to care, and resilience — and replace them with frameworks that name oppression and demand accountability
- Understand racial health inequities across cardiovascular, infectious disease, pulmonary, and cancer medicine — and bring that knowledge into your mental health practice
- Advocate for patients navigating a healthcare system that is simultaneously failing and harming them
What separates Advocate+ from Advocate: the focus shifts from historical orientation to present-tense analysis. You are no longer learning what happened. You are learning what is happening — and developing the analytical tools to see it clearly enough to act.
THIS IS NOT ABOUT COMFORT.
Advocate+ will ask you to sit with the weight of what racism is doing right now — not in the past, not in theory. It will ask you to look at systems you work within, benefit from, and are implicated in. It will ask you to stay when that becomes uncomfortable.
The Protective Care Framework — the clinical and ethical foundation of this entire fellowship — becomes executable in Shield. But you cannot execute what you cannot see. Advocate+ is what makes it visible.
